Design Request

This form should be used to submit requests for design jobs that are digital or that will not be printed on campus. Examples include: digital presentations, digital signage, social and digital media graphics, swag (bags, water bottles, stickers, etc.), metal signage, banners, t-shirt designs, etc.

The timeline for these requests may vary depending the format of the project

If you need to see a proof of your request before the final product is sent to you/an off campus vendor please fill out this date. We need a minimum of 4 business days between date of submission on this date for small projects.

What is the last day this project can be worked on by our team before the final product needs to be sent to you/an off campus vendor.

Please Note:

Fees will be applied to projects submitted with less than four business days between the time of submission and the due date.

By checking this box, you are confirming that the content you are providing upon the submission of your request is final (already edited and approved by you and/or your team members).

If the content you are providing is not final, please refer to our list of fees for information on how we charge for rounds of revisions.

By checking this box, I agree that any project that exceeds three rounds of revisions will be charged $150 per hour.

Example: If you request a fourth revision to your project and it takes 20 minutes to complete the additional work requested it would be a fee of $50.
